Service Description

The Heart Disease and Stroke Prevention service works with health systems to prevent and manage heart disease and stroke among North Carolinians. Activities include:

 
  • Promoting the adoption and use of electronic health records within health care systems
  • Promoting clinical guidelines for blood pressure control and cholesterol management
  • Implementing team-based care for patients with high blood pressure and high blood cholesterol
  • Linking community resources and clinical services that support systematic referrals, self-management, and lifestyle change for patients with high blood pressure and high blood cholesterol    
  • Collaborating with the Stroke Advisory Committee and the Justus-Warren Heart Disease and Stroke Prevention Task Force
