Case Management and Counseling, provided by local county departments of social services, offers an array of services that help individuals and families live independently in the community, or when the situation requires, arranges for services to remove someone from a harmful situation. The array of activities includes:
- Health Support services to identify health problems and appropriate treatment;
- Personal and Family Counseling to facilitate problem solving and conflict resolution;
- Individual and Family Adjustment services to support overcoming issues such as homelessness, school difficulties, substance abuse, and family abuse or neglect;
- Representative Payee services to manage money for people who are unable to it for themselves;
- Employment and Training services to help people prepare for work;
- Adult Placement services to find alternative living arrangements when adults are at-risk for harm and unable to remain in current living situation;
- Case Management Services to coordinate all of the above listed services.
The Adult Home Specialist Fund, also part of the Case Management and Counseling Service, supports the efforts of the county departments of social services to monitor the adult care homes to ensure licensed adult care and family care facilities meet state requirements and are providing care that meets the needs of the residents.
