Service Description
|
Community Focus Infant Mortality provides services for women and their infants with a specific focus on African-American and Native American families in certain North Carolina counties and communities. Services include: - Outreach, case management and health education before, during and after pregnancy to improve health outcomes.
- Supportive services for women and their children for two years after delivery.
- Depression screening/referral
- Interconceptional care coordination
- Reproductive life planning
- Infant safe sleep practices
- Tobacco cessation and prevention
These programs include Baby Love Plus, Infant Mortality Reduction and Healthy Beginnings and are in local health departments and community based organizations across the state.
